Core Leader requirements

Ensure that any incident involving a caregiver hospitalization or serious
injury at work is reported immediately to CorVel. For medical events that
occur at work, but it is not clear if the cause is work-related, please
contact your ministry's
Sr. Workers Compensation Consultant for additional guidance on OSHA reporting requirements.

Investigate all caregiver incidents as soon as possible but no later than
72 hours following notification of Accident Investigation Review (AIR)

Identify and help address the root cause

Provide information to the third-party administrator, CorVel when requested
Following the above process will:

Simplify and expedite reporting

Support injured caregivers

Help build and sustain a culture of safety (high reliability)

Ensure OSHA compliance

For volunteers and non-employed worksite injuries, or caregivers that do
not have Caregiver Portal access, please contact the assigned Senior Workers
Compensation Consultant to assist with reporting.

OSHA logs and worker compensation incident data requests
The Workers’ Compensation team maintains and distributes OSHA compliance
forms, filings and provides workers compensation incident data upon request.

To request OSHA Forms (OSHA 300 logs, 300a annual summaries, 301 forms,
Sharps logs) please submit a written request to your ministry Senior Workers
Compensation Consultant that includes the specific purpose of requesting
this information and intended recipients.

To request W/C incident data (loss runs, claim status, return to work questions,
etc.) please contact your ministry's Workers Compensation Consultant.
