Getting Started

1. Enroll in Personify Health by clicking the Login/Enroll button below. Note: If you are enrolling from the app, enter choosewell for the Sponsor Code.

2. Are you a new caregiver? Schedule your no cost Next Steps Consult by clicking on the Next Steps Consult button below. It will take you to the Health section of your Personify Health account.

3. Download the Personify Health app. Access it all from the Personify Health mobile app — available for Apple and Android devices.
